MEDICAL APPLICATIONS (click here):

All athletes must have a current medical application on file with Special Olympics Illinois by a set deadline prior to each event. Medical applications must be signed by the parent/guardian and the athletes physician. Only original documents will be accepted, no photocopies or faxes. Medical applications must be updated every two years.
