The Illinois 5 Essentials Parent Survey.
All MVTHS students and teachers will be taking the 5Essentials Survey, which is a statewide survey designed to generate a detailed picture of the inner workings of your child’s school.
There is also a parent survey to be completed by parents/guardians of MVTHS students. Your participation in the survey will help us understand more about MVTHS and help guide improvement.
The survey takes approximately 15 minutes to complete and is completely confidential and will never be connected to you or your child. Only one parent/guardian per household should submit a survey for a given child.

Go to survey.5-essentials.org/illinois and select Parent Survey.
Enter requested demographic information.
Advance to the survey questions.
Click Submit Survey when completed.
